Job SummaryFacilitates the delivery of quality, patient-centered care for all program participants and coordinates servicesfor patients with abnormal results requiring additional evaluation.

Ensures that care is designed to meetindividualized patient outcomes.Minimum QualificationsEducation/TrainingBachelors degree in nursing.Experience5 years clinical experience (RN experience) in a related field.License/Certification/RegistrationValid RN license in the State of Maryland.

Certification in area of specialty preferred.Knowledge, Skills & AbilitiesResearch data collection and analysis skills.

Ability to use computer to collect data and preparereports.

Verbal and written communication skills.

Bi-lingual in Spanish preferred.Primary Duties and ResponsibilitiesCollaborates with program providers to develop and coordinate the plan of care from results to diagnosis and/or treatment.Communicates regularly with program providers and supervisor regarding patient responses to plan of care, identification of problems, and specific patients compliance or lack thereof.

Follows patients to a conclusion as determined by State clinical and time guidelines.Contributes to evaluation of Critical Failure List.Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards.

Complies with governmental and accreditation regulations.

Participates in external reviews by the State, TJC, and other accrediting groups.

Demonstrates competency in area of specialty to meet age specific, biopsychosocial, and spiritual needs of patients served.Disseminates and applies knowledge in order to meet the educational needs of the health care team, community, patients and families.Evaluates and documents the patients response to the plan of care and achievement of outcomes.Generates written and certified notices when appropriate.

Communicates with patient, family and/or significant other to identify and clarify patient responsibilities and resources available to assist the patient financially with diagnosis and treatment.Makes recommendation for modifications to the plan of care as indicated.

Maintains care plan format as provided by State DHMH.

Performs home visits on select patients.

Directs home visits for Outreach staff.

Manages own professional growth in the area of managed care, care management, other health care, financial trends, clinical practice and research.Manages patient care according to minimal clinical elements, and/or multidisciplinary plan of care, by directing decision making and identifying and managing barriers that impact on patient care outcomes.Manages patients from time of initial screening results to close out.

Manages patients with abnormal results to diagnostic or treatment disposition dates.

Identifies essential resources needed to implement the plan of care.Monitors Patient Navigation clients by: verifying insurance coverage for procedures; correctly coding on demographic sheets; conferring with fiscal coordinator; identifying barriers; enters appropriate clients in the Care 2 Care database.Participates in multidisciplinary quality and service improvement teams as appropriate.

Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate.Performs other duties as assigned.Receives and reviews all patient results.

Performs a comprehensive assessment incorporating information obtained from demographic enrollment data, test results, and recommendations from program providers to identify patient-specific problems or needs related to diagnosis and treatment.Refers patients to State programs that provide funding for additional diagnostic and treatment services.

Communicates with primary and program providers as well.Serves as consultant in area of expertise for patients, staff and community.

Collaborates with other program case manager, program staff, and provider's staff.
